Covered Services
- Nursing home - a licensed nursing home that provides skilled, intermediate, custodial or physical restoration care services.
- Adult Day Care
- Assisted Living Facility
- Home and Community Based Services- care received at home, in a home hospice or adult day care setting.
Benefit Eligibility Overview
An insured is eligible for benefits when they are unable to do 2 of 6 Activities of Daily Living (ADLs) Impairment: Bathing, dressing, Continence, Transferring, Eating and Toileting
OR
A Severe Cognitive Impairment (includes Alzheimer’s Disease)
